| A.
The home shall accept only those persons who are: |
1.
Mobile and can self evacuate; |
2.
Able to initiate and accomplish most activities of daily
living but may require supervision or physical
assistance; |
3.
Not in need of licensed or professional nursing care or
monitoring except for temporary episodic illness. |

| B.
For persons admitted to the home for short term
residential care, the following shall be required: |
1.
A health examination shall be conducted by a licensed
practitioner 30 days prior to admission which shall
include any orders and medications required by the
resident; |
2.
An assessment focused on the services needed by the
resident; |
3.
An admission agreement, which shall list the services
which shall be provided during the time of residence. |

| C.
For the residents who intend to live at the facility for
more than a short term the following shall be required: |
1.
A health examination shall be conducted within 30 days
of being admitted to the home which shall include; |
 a.
medical requirements; |
 b.
functional activities and limitations; |
 c.
medication needs and orders; and |
 d.
dietary needs |
2.
The administrator and/or designee, in conjunction with
the resident's licensed practitioner, and the resident's
family, resident's personal representative or guardian,
shall complete an initial resident assessment as set
forth in He-P 804.06 (j), prior to admittance; |
3.
The administrator shall insure that resident assessments
shall be completed at least every six months after the
initial assessment. |
4.
The residential care home shall complete an admission
agreement with the resident which shall include: |
 a.
a listing of services which shall be provided; |
 b.
a listing of services which shall be in addition to the
services required by He-P 804.06 (a) and for which
additional payment shall be required; |
 c.
information on the home's rules for the residents; |
 d.
information on the policy and procedures for policy on
reserving a place in the home when resident is
hospitalized or out of the home for a period of time; |
 e.
grounds for termination agreement; |
 f.
the notification required for involuntary transfer for
reasons other than emergency situations. |

| D.
The home shall not accept any resident whose assessment
indicates services required which the facility cannot
provide. |
